Privacy notice
This notice covers this website. It does not cover your clinical record, which is held separately under a different set of rules and is explained to you in writing before your first session.
What this website collects
Only what you type into a form on it: your name, your email address, a phone number if you give one, and the practical notes you write. Nothing else is gathered about you. We do not run advertising trackers, we do not sell or share anything with data brokers, and there is no third-party pixel on any page of this site.
What our forms deliberately do not ask
No form on this site asks about symptoms, diagnosis, medication or treatment history, and we ask you not to volunteer them. A web form is ordinary email infrastructure, not a clinical record system, and information of that kind is safer in a consultation than in an inbox. If you send it anyway we will not copy it into your record — we will delete the message once we have replied.
Where enquiries go
Form submissions reach the practice's intake team by email and are held in the practice's customer system while the enquiry is open. Enquiries that do not become clients are deleted after twelve months. Enquiries that do become clients are superseded by your clinical record and the web enquiry is deleted.
Your clinical record is separate
Once you are a client, notes about your care are held under professional record-keeping obligations and applicable health-privacy law, with their own retention periods, their own access route and their own consent process. That is set out in the client agreement you sign at the first session, not here.
Cookies
This site sets no advertising or analytics cookies by default. If the practice later turns on visitor analytics, this notice will say so and a consent banner will appear before anything is set.
Your rights
You can ask what we hold about you, ask for it to be corrected, or ask for it to be deleted, and we will answer within thirty days. For website enquiries, write to the address below. For anything in a clinical record, ask your clinician — the process is different and they will walk you through it.
Contact
Write to privacy@thornfield.example or to the practice manager at 1140 SE Belmont Street, Portland, Oregon 97214. If you are not satisfied with our answer you can complain to the relevant state or federal regulator, and we will give you their details on request rather than making you look for them.
